Catherine’s Story (Consultant Anaesthetist)

Catherine describes the challenges she experienced in communicating the incident to the patient’s family. She thinks that formal debriefs following incidents should be conducted to help staff understand what has happened and to introduce changes to prevent similar incidents occurring in the future (9mins 32secs)
